Shure is offering rebates of up to $1000 for the trade-in of Shure 700 MHz frequency band (698 -806 MHz) wireless systems and related components purchased before February 1, 2007 and for any other manufacturers' qualifying 700 MHz frequency band wireless systems and related components.  For information on Shure wireless systems purchased on or after February 1, 2007, please contact (800) 25-SHURE (800-257-4873). This rebate offer is subject to all of the terms and conditions specified in this Form.

3  Form must be signed by Purchaser. If Purchaser is a corporation or entity other than an individual, Form must be signed by an authorized officer or agent on behalf of Purchaser.
4  By signing this Form, Purchaser certifies to Shure that all information submitted to Shure by or on behalf of Purchaser in connection with this rebate offer is true, correct and complete.

To Receive Your Rebate Send:

• Your original dated sales receipt (no photocopied substitutions) for your newly-purchased qualifying Shure wireless system(s)  and, if applicable, qualifying Shure component(s), issued by the authorized  Shure dealer where the product(s) was (were) purchased, indicating model number(s) and name and address of the dealer;
• The actual (no photocopied substitutions) model number label (UPC proof of purchase) from the end of each product carton of the newly-purchased qualifying Shure products;
• Both the receiver(s) and transmitter(s) of Shure or other manufacturer's qualifying trade-in wireless system(s) in the 700 MHz frequency band and, if applicable, qualifying trade-in components; and
• This completed and signed Rebate Offer Form to Shure at:

Additional Terms and Conditions:

This rebate offer is made exclusively by Shure Incorporated (Shure). This rebate offer applies only to qualifying trade-in wireless systems and related components, whether manufactured by Shure or other manufacturers, which were designed and enabled to operate in the so-called 700 MHz frequency band (i.e., any frequencies from and including 698 MHz through 806MHz). The rebate applies only to new Shure wireless systems models UR14D, UR24D/SM58, UR14S, UR24S/SM58, UR1, UR2/SM58, UR4S, UR4D, ULXP14, ULXP24/58, ULXS14, ULXS24/58, ULX1, ULX2/58, ULXS4, ULXP4, MX690, MX890, SLX4L, P7TR, P7T, P7R, PGX14, and PGX24/SM58, together with  the specified new Shure wireless system components. This rebate offer is available only to original end user purchasers and is valid only for their purchases of the qualifying new products made at Shure-authorized retail stores in the United States between December 1, 2008 and May 31, 2009 (and in any event excludes any products in the 700 MHz band). Distributors, retailers, and other resellers of Shure products, together with their employees, agents and families, are not eligible for rebates, nor may Shure product distributors, retailers, resellers, or their employees, agents or families claim rebates on behalf of end user purchasers. Applicable rebate amount specified above is for each qualifying new Shure wireless system and, if applicable, Shure component purchased, provided that a rebate claim is properly made in accordance with this offer (including a proper trade-in for each Shure system or component purchased). Rebate claims must be postmarked no later than June 30, 2009 and received by Shure no later than July 15, 2009. Rebate claims that do not include all of the proper documentation and all of the other materials required to be submitted will be ineligible for the rebate. Proper documentation and other materials include: the properly completed and signed official Rebate Offer Form; actual [no photocopied substitutions] model number label [UPC proof of purchase] from end of cartons; original dated sales receipt issued by the authorized Shure dealer at the time of original sale, with the dealer's name and other identifying information; and qualifying trade-in Shure or other manufacturer?s wireless system(s) and, if applicable, component(s).  Completed Rebate Offer Form must include all required information, including, but not limited to, product quantities and, where applicable, brand and system information. Shure reserves the right to require additional information from rebate claimants to ensure compliance with the terms and conditions of this rebate offer. All submitted documentation and materials become the exclusive property of Shure and will not be returned. Shure is not responsible for late, lost, damaged, misdirected, incorrect, incomplete, illegible or postage due rebate claims or claims that are not properly submitted for any reason. Such rebate claims need not be honored. Keep copies of documentation submitted for your records. Offer open to U.S. residents only. This rebate offer is void where taxed or prohibited by law.  In any event, rebate recipients are solely responsible for any taxes imposed on their rebates.  Rebate recipients should allow at least 6 to 8 weeks for Shure to send their rebate checks. Shure reserves the right to reject wireless systems or components submitted to Shure as part of a rebate claim if Shure determines that any part of the systems or components were not in working condition at the time of their submission or the systems or components were not genuine product acquired by the end user purchaser in a bone fide sale (e.g., counterfeit products). Shure also reserves the right at any time, without notice, to suspend, terminate, or alter in any respect this rebate offer or any related terms or conditions.  Shure decisions regarding rebate eligibility and other matters related to this rebate offer are final. Shure rejection of any improper rebate claim will not entitle the claimant to any refund, return or other accommodation with respect to the claimant?s purchase of new Shure wireless systems or components.
